You are probably wondering how you manage to get one of these boxes?
Please remember to leave your full name AND full address including postcode. When the questionnaire is finished we will receive an email with your address on it which we then print out and put on a box.
Due to a high percentage of boxes being returned undelivered we now require that £3 be paid to cover the cost of the Postage and Packaging.
Boxes can take up to 6 weeks for delivery. If you havnt received your box by 4 weeks please send us a message to make sure we have definately received your request.
Boxes are for people in the UK only, with mental health issues.
